Structure and mechanism
SS-31 (elamipretide) is a tetrapeptide with an alternating aromatic-cationic motif, and that architecture is what gives it its defining property: it concentrates at the inner mitochondrial membrane without depending on the membrane potential that most mitochondrial probes rely on. Once there it associates with cardiolipin, the signature phospholipid of the inner membrane.
In preclinical laboratory models the research questions follow from that binding — how cardiolipin association affects cristae architecture, what happens to electron-transport efficiency and ATP synthesis when the membrane's lipid environment is stabilized, and how reactive oxygen species behave under ischemia-reperfusion conditions. It was the first compound in the SS (Szeto-Schiller) peptide class, developed at Weill Cornell in the early 2000s.
